there are no file names just use offzip.
offzip -a -z -15 c:\file.dat c:\extract 0
you can specify the -1 option if you want it to combine the file into one

the bone indecies are floats you need to divide by 3 or 4 i forget what one.
i dont think there was anything wierd on the weights.

ps3 or 360 files? it also looks extracted wrong. the models that convert are extracted correctly. you did offzip.exe -1 -a -z -15 c:\file.bin c:\extract 0 ? i don't have this game so i cant say. also they might of started using a 2nd compression on top of zlib after 7 i am not sure but most of the f...

just click start then click run then in the run box type cmd hit enter now drag xbdecompress into the black box that showed up now add a space now drag the file you want to decompress now add another space now drag that same file into the black box again and add .uncompressed after it hit enter now ...

yeah that solves the problem also note that the face sections i labeled are the lod's you only need to import the first one to get the best version of the model.
