Are you using the latest version of vgmstream?

Because all I did was rename the extension and it played it back fine as it is only 2 channels.

Do you have the latest version of FSBExt from Aluigi?

I just ran the file through FSBExt just then and it gave me 2,462 files with the GENH extension.

These can be played with a Winamp plugin known as VGMStream.

You need to adjust the header of the files before ToWAV will convert it from XMA to WAV. You will need the following: 1. QuickBMS from Aluigi 2. AlphaTwentyThree's XMA Transform Script 3. Dark Souls on the Xbox 360 doesn't use XMA. It uses PCM WAV IIRC :)
