Great work. (duplicate file paths in .index files is strange)

Try to use with RAGE, but it fails. As I understand, the reason in another archive method, which probably works directly with streamed.resources.

No, because no one made .streamed files unpacker and find a way to get filenames.

ww2ogg can convert unsupported WEMs without packed_codebooks.bin and packed_codebooks_aoTuV_603.bin, but result oggs unplayable and revorb.exe cant fix them.

I think, possible to unpack .streamed data without a header in another file, because .streamed files of Evil Within can be unpacked in any version(PC, PS3, Xbox360).
